Job Summary:
We have partnered with Demelza who offer expert clinical and emotional support for children and young people with life-limiting and life threatening conditions, whilst supporting their family.
This role will work closely with children, offering truly one to one care.
Seeking candidates to deliver comprehensive nursing care and support to children and young people with life-limiting or life-threatening conditions, as well as their families and carers. This involves applying the expertise, knowledge, and skills of a qualified practitioner across diverse situations, operating independently while contributing to and drawing from the wider team’s expertise. The role includes collaborating with other services to ensure the needs of the child or young person and their family are met effectively.
Key Responsibilities:
Care Delivery: Assess, plan, deliver, and evaluate high-quality nursing care tailored to the individual needs of children, young people, and their families. This includes care plans, symptom management, end-of-life care, safeguarding, and liaising with external professionals.
Team Collaboration: Support team members to ensure care plans meet agreed standards and contribute to shared knowledge.
Case Management: Lead in managing caseloads for assigned children and families and take responsibility for delegated specialisms.
Clinical Leadership: Identify and act on significant changes in patient conditions and deliver prescribed care, including end-of-life care and hospice service referrals.
Professional Relationships: Foster strong communication with children, families, and colleagues, addressing complex situations, conflict, and difficult conversations effectively.
Documentation: Maintain accurate records in line with Nursing Midwifery Council standards and legislation.
Post-Death Care: Lead post-death care and provide family support during bereavement.
Advocacy: Represent children’s best interests in multidisciplinary meetings, focusing on symptom management, end-of-life care, and safeguarding.
Flexibility: Work flexibly, manage time effectively, and participate in on-call rotations.
Continuous Development: Engage in supervision and learning opportunities to enhance professional practice.
Integration: Promote collaboration across hospice sites to ensure consistent and effective care delivery.
Essential Requirements:
Qualified Paediatric Nurse or Learning Disabilities Nurse with an active NMC pin
Strong communication and problem-solving abilities
Flexible to meet the needs of the service
